This indicates that the pillow does not support the cervical spine in a neutral position, leaving the head ergonomically too low.
Sleeping with a hand under the pillow is an attempt to increase pillow height and correct head position. This sleeping posture is not ergonomic and can unnecessarily wake the sleeper. It may also cause numbness in the hand as nerves and blood vessels become compressed. An ergonomic pillow that conforms to body heat supports the neck significantly better than a fiber-filled pillow.
If you have difficulty finding a suitable pillow, the cause may also be a poor mattress: for example, when sleeping on your side, your shoulder may not sink into the mattress enough — in other words, the mattress may be too low or too firm. Replacing a topper may help in some cases, but it is also worth carefully evaluating the condition and suitability of the base mattress for your body.
